Students who need to cancel their housing need to log into MyHousing and visit Housing Forms. |
| Study Abroad/Disney Co-Op/Internship Cancellation |
Completed when the student is canceling one semester of their academic year contract due to study abroad, Disney co-op or internship. Must be completed and returned with a single student cancellation form. |
| Family Housing Cancellation |
Completed at least 30 days before check-out date. This form is only for family housing residents. |
| Family Housing Childcare Provider Information Form |
This form is to allow family residents to have established childcare providers without having to retrieve a separate guest parking pass from the Cougar Village Commons every time their provider comes. |
| Residents Who Require a Nurse or Attendant |
Residents who require a nurse or attendant should complete a Nurse/Attendant Information Form for each nurse/attendant who will need access to your room/apartment. Submit the completed form to Rex Jackson, Associate Director of Residence Life, SIUE Campus Box 1056. Please allow two business days for processing. |
| Gender Inclusive Housing Agreement |
This form is for upperclass students who want to live in gender-inclusive housing. The completed form should be returned to your Community Director. |
| Address Change |
Students wishing to change their address online should visit CougarNet or visit the SIUE Registrar for an address change form. |
