Name	Description
Registration - Add-Drop Form	Add or drop courses in the Service Center.
Registration - Withdrawal Form	Withdraw completely from the University.
Address Change Form	Update student’s permanent address.
Copy of Final Grades Report Form	Formal grade report of current term grades (processed at the end of the term).
Directory Information Release Form	Objection to the release of directory information to the public.
Dual Enrollment FERPA Release	For dual enrollment students: SIUE cannot report your grades to your high school until you submit this release authorizing us to do so. Authorization must be renewed every semester.
Health Experience - Pre-Approved Option	Use if you have met the health experience requirement by fulfilling one of the pre-approved options.
Health Experience - Not on Pre-Approved Option	Requesting a non-course option to meet the health experience requirement, and it is not on the pre-approved list.
Health Experience - Transfer Course	Requesting a transfer credit course to meet the health experience requirement.
Graduate Student Request Form	Request transfer credit, apply credit older than the time limit for degree program, enroll in graduate-level courses for graduate credit while an undergraduate student, additional thesis credit, and continuous enrollment.
Intercampus Registration - SIU System	SIU System offers an intercampus registration for students attending SIUE or SIUC. This Intercampus registration allows students from SIUE to register for courses on SIUC campus.
Name Change Request Form	Change your legal name, add an additional name to your student record, and update your emergency contact information.
Personal Data Changes	Update birthdate, gender, ethnicity, correct SSN number, update emergency contact information, specify religious preference.
Reclassification of Residency Form	Update out-of-state residency determination to in-state status.
Reinstatement Request	Petition for reinstatement after one semester of non-enrollment as a result of two academic suspensions.
Replacement Diploma Request	Order form for a replacement diploma.
Student Petition Request (Undergraduate)	Submit with appropriate signature approvals to allow adjustments to student course record in Banner and Degree Works including, but not limited to, Military Credit Re-Evaluation, Senior with Degree Evaluation, Substitution for Requirement, Transfer Credit Re-Evaluation, or Waiver of a Requirement.
Student Petition Instructions (Undergraduate)	Instructions to complete a Student Petition request. This allows adjustments to student course record in Banner and Degree Works including, but not limited to, Military Credit Re-Evaluation, Senior with Degree Evaluation, Substitution for Requirement, Transfer Credit Re-Evaluation, or Waiver of a Requirement.
Student Verification Form	The Office of the Registrar can provide proof of enrollment at SIUE for reasons including but not limited to, health insurance, loan deferment or tax purposes.
GI Bill Certification Request Form (electronic fillable version)	For students who receive GI Bill benefits, students must report enrolled courses each semester to receive benefits. This is the online electronic fillable version.
Withdrawal and Tuition Appeal Form	Request for exception to the withdrawal deadlines/and or tuition refund policies based on military assignment, death of immediate family member, unexpected financial hardship, or sudden loss of transportation within the term
School of Business Visiting Student Course Verification Form	Request to take business courses at SIUE transfer back to their home institution
