Religious and Medical Exemption Requests From Mandated Vaccination(s)
The majority of our clinical placement sites have Federal and State mandates in place regarding mandatory vaccinations. If you wish to be exempt from vaccinations due to religious beliefs, a documented exemption must be on file in order to maintain compliance with these mandates. Requests for religious exemptions will be reviewed based on the information provided. The University may require the submission of additional documentation or may need to obtain additional information concerning the identified religious belief, practice or observation from a religious leader or scholar.

Pregnancy Accommodation for Students
Pregnancy/Parental Accommodation Request Form (electronic intake) - It is the goal of SIUE to support pregnant mothers and families to help mitigate any disruption to their educational and career goals while preserving the essential elements of the academic program/position. If you are pregnant, recovering from childbirth or have a condition related to pregnancy, you have the right to ask for a reasonable accommodation. What is the pregnancy accommodation process?
Once you’ve completed the Pregnancy/Parental Accommodation Request Form (electronic intake), you will be contacted by the Director for Equal Opportunity, Access and Title IX Coordination (EOA), for a one-on-one meeting.
What types of accommodations may I request?
Common examples of reasonable accommodations for pregnancy or pregnancy-related conditions may include, but are not limited to:
- allowing breaks and frequent trips to the restroom during class, as needed
- flexibility with attendance including excused absences due to pregnancy or related conditions as long as the doctor says it’s necessary
- medical leave of absence, including allowing the student to return to the same academic and extracurricular status as before the medical leave began
- assistance with heavy lifting
- a private space for expressing milk
- providing alternatives to make up missed work due to pregnancy or childbirth
- parking and transportation issues
- flexibility in the administration of exams (including rescheduling and extensions)
- providing pregnant students with the same special services as other students with temporary medical conditions including independent study, remote classrooms, or other similar types of academic adjustments
- allowing the student to continue participating in off-campus programs such as internships, career rotations

Lactation Spaces
Lactation Spaces are available for students, employees, and visitors. The lactation rooms are designed to provide a clean and private space for pumping or expressing breastmilk conveniently located near academic and workspaces. The permanent lactation spaces are furnished with a comfortable chair, refrigerator, side table, and where possible a sink.
There are three permanent lactation spaces currently available:
- Morris University Center
This lactation space is located inside the “green room” inside the southeast entrance (near Starbucks) to the left, adjacent to the restrooms. To gain access to this room, one must request the key from the MUC help desk located across from the Cougar Bookstore. The help desk is open during fall and spring semesters 7 a.m. to 10 p.m. and during the summer 8 a.m. to 6 p.m. For more information you can call 618-650-5555
- Peck Hall (PH 1414)
To use the Peck Hall lactation room, one may check out a key from Facilities Management’s Peck Hall office located in PH. If no one is present there, contact 618-650-3711 or 618-650-3126 to request an operating engineer to unlock the door.
- Vadalabene Center
The room in Vadalabene Center is room 1120 (located near equipment issue); this is a nursing room/changing room. Next door to this room is two family friendly/gender-neutral restrooms. The new Health Sciences Building will also have a full lactation room with a sink, which will bring the total number of dedicated lactation rooms on campus to four.
