Protocol Submission Guidelines
All research and/or instruction, including field activities, involving the use of live vertebrate animals, must be reviewed and approved by the Institutional Animal Care and Use Committee (IACUC) before the research begins and before any animals are brought to the SIUE campus.
STEP 1 Training
Before submitting an animal care protocol, complete the CITI Online Training and Certification for Animal Care. All faculty, staff, and students using animals in research or instruction must complete the course titled "Working with the IACUC." Please also choose the appropriate species or procedure specific course depending on your work or interests (mice, rats, etc.).
STEP 2 Protocol
Complete the IACUC Animal Care Protocol through the protocol module of Kuali Research. You can view a tutorial on submitting an IACUC protocol in Kuali Research under User Instructions.
The deadline to submit an animal care protocol to the IACUC is always the first Wednesday of the month in which you would like for it to be considered.

- Waiver & Release of Liability, & Covenant Not to Sue - For those conducting animal research/observation in the field (not in the lab at SIUE) you must sign and submit the Waiver & Release of Liability, & Covenant Not to Sue form to the Graduate School before you begin your study.
- Occupational Health and Safety Program for SIUE Faculty, Staff and Students in Contact with Animals

- As part of the SIUE Occupational Health and Safety Program for the care and use of animals in research, all SIUE faculty, staff and students who have, or plan to have, regular contact with live vertebrate laboratory or wild animals, are required to have a current medical history, physical examination, and appropriate vaccinations/testing prior to exposure to the animals. These requirements are in response to rules set forth by the Office of Laboratory Animal Welfare (OLAW), an agency within the U.S. Department of Health and Human Services.
- Health History Evaluation and Vaccination History form. A portion of this form must be completed and signed by your doctor or licensed health care provider (includes SIUE Health Services, if you are a student). The remainder of the form must be completed and signed by you. If you have not received the required vaccinations, go to your doctor or healthcare professional to get the vaccinations.

Note: For students using the SIUE Health Services, a physical exam may be required at the student’s expense.

- Letter of Explanation for your Doctor - Give a copy of this letter to your doctor or licensed health care provider.
- Vaccination Declination - This form should only be used if you decline the vaccinations.

STEP 4 Ordering Animals
After you have received IACUC approval on your protocol (Step 2) you can order animals. In addition to other required signatures, your purchase requisition form must be signed by the Office of Research & Projects on behalf of the IACUC. You may submit your purchase requisititons to the Compliance Specialist for signature by sending the documents to Box 1046 or emailing the documents to researchcompliance@siue.edu.
Please note: you cannot order animals if you have not recieved IACUC approval.
