Complete and attach the Notice of Employment Opportunities, advertisements and their invoice voucher, purchase requisition, or indication of payment by p-card to the recruitment routing slip. Forward all of this information to be approved for posting as indicated on the recruitment routing slip. - Applicant Data Request [pdf]
As applicants send information required to apply for the job, provide each of them with an Applicant Data Request form to be returned to the Office of Equal Opportunity, Access and Title IX Coordination. - Applicant Address Summary [pdf]
Once all applications have been received in this office, the hiring unit must then provide the Office of Equal Opportunity, Access and Title IX Coordination with the names of all applicants who were sent this form by completing the Applicant Address Summary form. - Affirmative Action Recruitment History [pdf]
