Who can be seen in health service?
What is the cost?
Students who are assessed a health service fee in their tuition and fee payments will be seen by a member of our health care delivery team at no addition charge. A minimal fee is charged for procedures, laboratory work, pharmacy items, physical exams, and immunizations.
Do I need insurance coverage to attend SIUE?
SIUE Health Service advises students to have adequate health insurance. A reasonable student health insurance plan is available through Academic Health Plans.
Mandatory health insurance is required for international students who have an F-1 visa. International students are required to have Academic Health Plans.
The School of Nursing and School of Pharmacy require proof of health insurance. This is managed through the individual schools.
Academic Health Plan brochures and enrollment forms may be picked up in Health Service. You can also apply online by going to their website at My AHP Care.
Do I need an appointment to be seen in health service?
Yes. Appointments allow Health Service to provide the best care possible to all students. Also, appointments give students the ability to schedule for a time that fits their academic schedule.
Prior to your first visit to Health Service, you will be asked to complete three online forms (Health History Form, Notice of Privacy Practices, and Treatment Agreement). This is a one-time requirement and can be completed and submitted online at Cougar Care.
A pre-visit questionnaire is required for each visit specific to your condition. This also can be completed and submitted online at Cougar Care.
