Health Service Immunizations

The State of Illinois Department of Public Health mandates that all new students (first-year, transfer, visiting, or re-entering) must provide proof of protection against the diseases of Rubeola, Rubella, Mumps, Tetanus, Diphtheria, Pertussis, and Meningitis ACWY.
This requirement must be met on the 1st day of attendance. A $25 non-compliance late processing fee will be imposed for failure to comply. In addition to the fee, proof must be provided to register for subsequent semesters.
Proof of immunity can be provided by sending a copy of a signed and dated school immunization record (your high school record, military record, or child health immunization record) OR by having your family physician or health care provider complete the immunization form. The document submitted must include the facility or provider name, address, and telephone number. You may also receive the immunization in Health Service as an enrolled student.

Domestic Student Requirements
Tetanus/Diphtheria/Pertussis (DPT, DTP, DT, DTaP, Td or Tdap)
- Three doses are required
- One of the doses must be a Tdap
- One dose must have been given within 10 years of first date of term
- Tetanus Toxoid is NOT acceptable in fulfilling this requirement
Measles, Mumps and Rubella (MMR)
- Two doses at least 28 days apart and after 12/31/67
- First dose must be after 12 months of age
- OR, a combination of the following:
- RUBEOLA (Measles, Red Measles)
- Two doses at least 28 days apart
- First dose must be after 12 months of age
- MUMPS
- Two doses at least 28 days apart
- First dose must be after 12 months of age
- RUBELLA
- Two doses at least 28 days apart
- First dose must be after 12 months of age
- OR, positive titers confirming immunity (please attach lab report)
- RUBEOLA (Measles, Red Measles)
Meningococcal Vaccine (MenQuadfi, Menactra MCV4, Menomune MPSV4, Menveo or Meningococcal ACWY) Please note Meningitis B or Meningitis C vaccines are not acceptable. Learn more about Meningococcal Disease.
- Required for all new students under the age of 22
- One dose must have been given on or after 16th birthday
International Student Requirements
- All records not in English must be accompanied by a certified translation
- Must fulfill Domestic Student Requirements (as listed above)
- Tuberculosis (TB)
- All international students will complete a TB screening questionnaire prior to the 10th day of class or within 10 days of arrival to SIUE
- If upon review of the screening questionnaire the student is determined to be at-risk, the student must fulfill one of the following requirements:
- Quantiferon Gold (TB blood screen test) performed at SIUE Health Service
- OR, if a transfer student, show proof of a negative TB test, Quantiferon Gold, or T-Spot test performed at another U.S. university (must be no more than one year prior to SIUE semester start date)
- OR, provide documentation in English of prophylactic treatment of TB within or outside the U.S. in addition to completing a TB risk assessment at SIUE Health Service
Exemptions
- AGE - Students born prior to January 1, 1957, are exempt from the Measles, Mumps, and Rubella (MMR) requirement.
- MEDICAL Contraindications -- The student must present a written, signed and dated statement from a physician stating the specific vaccine(s) contraindicated and duration or medical condition that contraindicates the vaccine(s). Medical exemption forms are available from Health Service if needed or available on Forms.
III. PREGNANCY OR SUSPECTED PREGNANCY -- The student must present a signed and dated statement from a physician stating such. The Estimated Date of Delivery must be indicated.
- RELIGIOUS - The student must present a signed and dated Religious Exemption form (if the student is under the age of 18, their parent/guardian may sign on their behalf). Attached to the form should be a signed statement from the student supporting their religious beliefs.
Note: In accordance with public health recommendations, anyone with a vaccine exemption may be excluded from SIUE in the event of a disease outbreak.
