The following Clery Act forms are for use by authorized personnel only.
SIUE-Sponsored Student Travel Lodging Form
Used by University department heads and/or their designees to report University-sponsored student travel as required by the federal Jeanne Clery Act. Additional guidelines and instructions are included on the form.
SIUE-Sponsored Student Travel Lodging Form [pdf]CSA Clery Crime Reporting Form
Used by individuals who have been identified by the University as Campus Security Authorities (CSAs) as defined in the federal Jeanne Clery Act. A CSA should use this form to report any Clery reportable crime(s) that the CSA becomes aware of which occurred at Clery reportable location(s) as indicated on the form. In lieu of using this form, a CSA may call the SIUE Police Department and file a police report directly with University police. Additional guidelines and instructions are included on the form.
CSA Clery Crime Reporting Form [pdf]Year-End CSA Clery Crime Reporting Form
Used by persons identified by the University as being Campus Security Authorities (CSAs) as defined in the federal Clery Act. CSAs should use this form at the beginning of each calendar year to:
- report any Clery reportable crime(s) that occurred at Clery reportable location(s) that occurred in the previous calendar year but was not previously reported, or
- verify that the CSA does not have any additional Clery reportable crimes to report for the previous calendar year.

Year-End CSA Clery Crime Reporting Form [pdf]The Annual Security and Fire Safety Report (ASR)
The Annual Security and Fire Safety Report (ASR) provides information on crimes that occur on campus as well as fires at University owned student housing. In addition, the ASR provides information pertaining to the disciplinary process, university contact information, Illinois statutes, local agencies that may assist a victim of certain crimes, university sponsored crime prevention programs, and other pertinent information to assist the campus community.
