Admission Information
Implant Fellowship Application Requirements
To be considered for the Implant Fellowship, you must meet the following requirements:
- Must have graduated from an ADA accredited dental school.
- Must have National Board Certificate.
- Must have completed an ADA accredited program in a recognized specialty, GPR, or AEGD residency. Under extraordinary circumstances, private practice experience may be accepted in lieu of the above.
- Must have documented clinical experience in implant dentistry.
- Must hold or be eligible for an Illinois dental license and federal DEA license. Contact the Illinois Department of Professional Regulations for more information on how to obtain Illinois licensure: (217) 785-0800.
- Be either a U.S. citizen or U.S. permanent resident (possess a green card).
*Note: Holders of certain types of visas are not eligible for this program.
Implant fellows are considered employees of the university, not students.
In addition to meeting the other requirements listed here, you must be
either a U.S. citizen or permanent U.S. resident.

Application Items and Deadline
The Implant Fellowship Application and all additional items must be submitted by December 1 for the program that commences the following July 1.
Please submit all of the following items before December 1:
- Program Application
- Official National Board Scores Part I and II
- Letter of application (1-2 page essay about why you're applying to the program)
- Current Curriculum Vitae
- Official transcript forwarded from college(s)
- Official transcript forwarded from dental school
- Three (3) official letters of reference (one must be from Director of Advanced Program attended)
- Small photograph (2 in. x 2 in.)

SIUE Annual Security and Fire Safety Report
The SIUE Annual Security and Fire Safety Report is available online. The report contains campus safety and security information, crime statistics, fire safety policies, and fire statistics for the previous three calendar years. This report is published in compliance with Federal law, titled the “Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act” and the Higher Education Opportunity Act also known as the “Campus Fire Safety Right to Know.” For those without computer access, a paper copy of the report may be obtained, with a 24-hour notice, from the Office of the Vice Chancellor for Administration, Rendleman Hall, Room 2228, (618) 650‑2536.
