Families and children who live in St. Clair County, Illinois, are invited to apply to the SIUE Head Start/Early Head Start Program.

Please fill out this Intake Form
Requirements for enrollment include:
Certified Birth Certificate
- Head Start: 3 - 4 years of age; 5 years of age after September 1
- Early Head Start: 6 weeks - 3 years of age
- Expectant Families: Driver’s License, State ID, or Certified Birth Certificate
Documentation of Income
- Federal Income Tax Return
- W-2 Form
- No Income Statement
- Self-Declaration Letter
- Child Support Letter
- Letter from Social Security Administration
- Statement from the Department of Employment Security if zero income or unemployed
- Temporary Assistance for Needy Families (TANF)
- Supplemental Security Income (SSI)
- Foster Care Voucher
Mandated Health Requirements
- Current (within the past year) Physical Exam (3-5 year olds) or Well Baby Check (6 weeks-3 year olds)
- Current Immunizations
- Tuberculosis, Hemoglobin and Lead Results
