This short verification video gives an overview of the verification process, including what verification is, what documents may be required, and how to submit the documents.
2024-2025 Verification Forms are for anyone who filed the 2425 FAFSA between October 1, 2024-June 30, 2025, for the 2024-2025 academic year.
Satisfactory Academic Progress (SAP)
SAP Appeals Forms are to be used when a student’s financial aid has been terminated for GPA (less than 2.0), Completion Rate (less than 67.0%), or maximum hours (over 150% of published program length). There are deadlines for submission of new SAP Appeals each semester – see www.siue.edu/sap for these deadline dates and more explanation about Satisfactory Academic Progress.

If your financial aid eligibility has been terminated for GPA and/or Completion Rate, please complete and return this 3 page form to the Office of Student Financial Aid. This document includes the Academic Plan that must be completed and signed by your (student’s) academic advisor. | ||
If you have completed your first baccalaureate degree and are pursuing courses required for an initial teacher certification or recertification, not including endorsements, you may be eligible to receive student loans. This is the form you will complete and submit. |
