2026-2027 Verification Forms are for anyone who filed the 2026-2027 FAFSA between December 1, 2025-June 30, 2027, for the 2026-2027 academic year.
Check your CougarNet Financial Aid tab to verify the verification forms you need to submit. The Office of Student Financial Aid will only ask for the forms that are needed to complete verification.
Satisfactory Academic Progress (SAP) Appeal Forms
2025-2026 SAP Appeal Forms are to be used when a student’s financial aid has been terminated for GPA (less than 2.0), Completion Rate (less than 67.0%), or maximum hours (over 150% of published program length). There are deadlines for submission of new SAP Appeals each semester.
