Verification and SAP Appeal Forms
2025-2026 Verification Forms are for anyone who filed the 2025-2026 FAFSA between December 1, 2024-June 30, 2026, for the 2025-2026 academic year.
Satisfactory Academic Progress (SAP)
2025-2026 SAP Appeals Forms are to be used when a student’s financial aid has been terminated for GPA (less than 2.0), Completion Rate (less than 67.0%), or maximum hours (over 150% of published program length). There are deadlines for submission of new SAP Appeals each semester – see SAP for these deadline dates and more explanation about Satisfactory Academic Progress.
