School Physicals, Vaccinations, and Asthma Treatments.
Children over the age of 2 years and adolescents
School physicals
Physical examinations are provided for children, adolescents, and young adults. Students entering kindergarten, sixth, and ninth grades at an Illinois public or private school must provide proof of physical examination. More information regarding the states requirements can be found at the Illinois State Board of Education website.
Vaccinations
The WE CARE Clinic is a Vaccine for Children approved provider. Children ages 2-18 that meet eligibility criteria are able to receive their vaccines at no cost. Vaccines must be given in the clinic.
Immunization Schedules
Information about immunizations that are needed throughout the lifetime can be found by visiting the websites below.
- Customize Your Child's Immunization Schedule by going to the CDC site. You can easily check to see what vaccines your child should have. If not up to date, call for an appointment or come by any Friday for the specialty Vaccine Clinic.
- Catch Up Immunization Schedule for Ages 4 to 18 years is used by the WE CARE Clinic to be sure that your child is on track to be protected against diseases.
- 2018 Binational Immunization Resource Tool (Mexico & US) is a handy way for Mexican parents to know that vaccines your child had in Mexico are the same as in the U.S. Be sure you bring your vaccination record to the clinic to show when and where your child was vaccinated.
Asthma
For children who have been diagnosed with Asthma, parents and/or guardians can sign up to participate in the Asthma Trigger Assessment Program (ATAP). The Asthma Trigger Assessment Program (ATAP) is offered through the WE CARE Clinic and provides education and a assessments for asthma triggers in the home; households will also obtain an asthma action plan and additional resources for identified gaps to that may contribute to poor asthma control. .

Adults
Physicals, Immunizations, Women’s Health, STD Screenings, Health risk assessments and chronic illness.
Physical exams (all ages)
Adults are able to obtain physical examinations to work or to simply monitor their health. Physical exams and assessments are completed in the clinic. Call for an appointment at 618-482-6959.
Adult Immunizations
Adults never outgrow the need for vaccines. Vaccines for adults include the flu vaccine, tetanus, diptheria, pertussis, shingles, and pneumonia are available to our patients by appointment.
The CDC recommends that adults be vaccinated, especially if they have other health conditions which weaken their immune systems or have reached a specific age
Women's Health
The health needs of women and how they experience disease can be different in many ways than how men experience disease and because of this the WE CARE Clinic provides a host of health care services that are solely for women. Patients are able to obtain a well woman visit which includes a health and wellness screening for healthy heart and lungs, diabetes, gynecologic testing, counseling on contraception, and prescriptions for birth control.
Contraceptive Management
WE CARE Clinic is able to address the birth control needs for women of all ages. The We Care Clinic follows Women’s Preventive Services Guidelines and the American Congress of Obstetricians and Gynecologist guidelines for birth control for contraception management.
Paps smears and HPV testing
Pap Smears (Pap Test) are used to identify abnormal cells in the reproductive system, including the cervix and vagina. WE CARE Clinic provides this service to women. The Pap test looks for precancers or other infections. .
Sexually Transmitted Infection Screening and Treatment
The WE CARE Clinic offers confidential screening, treatment, and counselling for adolescents through adulthood.Confidential screening and treatment for most Sexually Transmitted Infections (STI or STD) are available for both men and women. If you are sexually active, having a test for a STD is important for your health. Health risk assessments and chronic illness
Men and women who know they are at risk for illnesses because of their health history or family history can learn ways to stay healthy. Also, adults who have been diagnosed with long term illnesses are able to use the WE CARE Clinic services to maintain their quality of life. Our health providers treat these and many other illness conditions.
Asthma
Treatment for adults with asthma follows the national recommendations to keep your airways open, breathe freely, and prevent asthma attacks. Resources for learning about asthma are offered and one to one education about asthma IS available. Stop in or call for appointment at 618-482-6959.
An Asthma Trigger Assessment Program (ATAP) is offered through the WE CARE Clinic. If you or any of your family or friends have difficulty controlling your asthma, call for help. An expert in asthma and home health will come to your home for free to help you identify potential triggers for asthma problems as well as provide personalized education that will help you stay well.

Diabetes
The WE CARE Clinic provides basic diabetes treatment and follow up using the guidelines available through the American Diabetes Association and the Center for Disease Control and Prevention. Patients who have been diagnosed with diabetes will be offered education about the chronic illness with regard to testing blood sugar, insulin injection/ medication management, nutrition, health coaching, and much more. The clinic believes that basic knowledge of diabetes helps improve your chances of diabetes control, prevention of problems, and quality of life.
High Blood Pressure and Heart Disease
Treatment for high blood pressure is one of the best ways to prevent stroke. At the WE CARE Clinic we provide a variety of interventions to assist with managing patients' high blood pressure. Patients are offered in-clinic services such as medication management, health coaching, and nutrition services.
Department of Transportation (DOT) drug testing (On Hold)
The WE CARE Clinic is an approved provider for DOT physical exams for commercial motor carriers (CDL holders), maritime crewmembers, pipeline workers, railroad workers, and transit workers. Individuals are able to call the clinic to set up an appointment.
Health Coaching
Assistance reaching health and wellness goals.
The Health Coaching Program at the WE CARE Clinic aims to assist with helping our adult patients reach their health and wellness goals. Our health coach assists our patients by supporting them as they navigate through their own personalized health and wellness plan. The health coach will meet as often as needed with their patients to assist them with the process of obtaining their goal.
The goal of the health coach is to assist the client with coming up with measurable, individualized goals that are important for them to obtain. The health coach will also help the client identify barriers that may prevent them from obtaining their goals while strategizing plans to eliminate those barriers.
Health coach is offered to all patients of the clinic that are 18 years old and older. Health coaching can also be conducted virtually.

Telehealth Program
The WE CARE Clinic has implemented a new Telehealth program for its patients. This program will allow some patients to conduct their providers appointment from the comfort of their homes with the help of technology.
Our Telehealth Program is a HIPPA compliant virtual health care service that allows video communication. The HIPPA compliant platform that the clinic uses is Zoom for Healthcare. Patients can use the Zoom platform with the smart phones or computers that have cameras, the camera will allow patients to see the provider and vise versa. During these sessions, patients are able to talk to their provider about their health concerns privately and obtain care without coming into the clinic.
This new Telehealth program is covered by most health insurance carriers without any additional costs.
How does this program work?
When a patient calls to schedule an appointment they should indicate that they would like to request a telehealth appointment. Their call and request will be transferred to the nurse who will speak to the patient about the appointment.
If the Telehealth appointment request is granted all new telehealth users will get a call from a clinic representative to help them sign-in to the Zoom platform.
On the day of the Telehealth appointment, you will get a call 30 minutes before your appointment. During this call, you will be triaged by clinic’s medical assistant. The information obtained will be uploaded to your file for the provider to review for the appointment.

Home Assessments
The Asthma Trigger Assessment Program helps patients gain control over their at-home asthma triggers.
Asthma Trigger Assessment Program
The Asthma Trigger Assessment Program assists patients with gaining control over their asthma triggers in their home. This program is a three part program that provides guidelines-based asthma care education for health professionals, asthma self-management and caregiver education, and in-home environment trigger assessments.
The purpose of this program is to:
- Identify patients with poorly controlled asthma.
- Ensure patients are receiving proper care for their symptoms and allergy testing.
- Provide asthma self-management education to the patient and caregiver(s).
- Provide in-home, environmental assessments to identify and reduce exposure to allergens and irritants.
- Provide education to local health professionals.
All WE CARE Clinic patients who have asthma or asthma related symptoms are able to enroll into this program. Participants who are interested in the program would have to agree to home visits from WE CARE Clinic Asthma Program staff member(s) to assess their in-home environmental asthma triggers.

ThinkFirst
A National Injury Prevention Foundation dedicated to preventing brain, spinal cord, and traumatic injuries through education, research, and advocacy.
The WE CARE Clinic is committed to doing its part in decreasing the number of unintentional injuries in the state of Illinois. ILLINOIS Department of Public Health (IDPH) states that unintentional injuries are in the fifth percentile when it comes to death amongst all age groups.
The WE CARE Clinic wanted to do its part in decreasing the number of unintentional injuries in the East St. Louis Community. So, the clinic partnered with the ThinkFirst Foundation and became one of their official chapters. ThinkFirst is a National Injury Prevention Foundation dedicated to preventing brain, spinal cord, and traumatic injuries through education, research, and advocacy. This program targets the whole family and works with community organizations, community leaders and other government stakeholders to spread awareness on how to prevent unintentional injuries by using evidence-based programming.
The WE CARE Clinic can provide a variety of educational presentations and programs for your school and community outreach events.
ThinkFirst! Kids
The “Street Smart” curriculum targets grades 1-3 and educates its audience about a range of topics which includes but does not limit bicycle and playground safety.
ThinkFirst! Youth
The “Safe Choices” curriculum targets grades 4-8 and educates its audiences about being confident about making safe choices on their own and with peers.
ThinkFirst! Teens
This curriculum provides presentations addressing the realities of life-altering injuries and the importance of making safe choices.
ThinkFirst! About Concussion
This curriculum educates youth and teens about the seriousness of concussions and the correlation that they have with brain injuries. This program could be suited for health classes and for those athletes prior to sports seasons.
ThinkFirst! To Prevent Falls
This curriculum could be for the older senior population. This class covers the basics of overcoming the fear of falling and taking steps to lower the risk of falling.
Case Management/ Therapeutic Services
An additional level of support to all patients who are in need of additional assistance.
One of the programs that the WE CARE Clinic offers is case management. Case management services are an additional level of support to all patients who are in need of additional assistance. The Case
Management services are collaborative with the patient, the patient’s treatment team, and other external resources to obtain assistance for the clients of the WE CARE Clinic.
Services are offered by our licensed clinical social worker who provides the following services:
- Mental health services
- Service referrals
- Care coordination
- Advocacy
- Assistance with advance directives
- Planning and executing care plans
- Home Visits
