Therapeutic Injections/Allergy Injections
Health Service provides a convenient service for continuation of injectable therapies. If you get regular therapeutic medication injections from a medical provider, under most circumstances, you can transfer your therapeutic medication injections. The prescribing medical provider will continue to manage your care and Health Service will serve as a temporary extension of your provider’s office while you are attending SIUE. You must provide documentation (outside records) from your current prescribing provider prior to scheduling an initial appointment. Prior approval is required, not guaranteed, and is on a case-by case-basis.
Prior to Your Initial Visit
Three forms are required before your first visit to our clinic. These forms may all be accessed on the Forms tab at CougarCare.
- Health History Form
- Notice of Privacy Practices
- Treatment Agreement
Review Patient Information for Therapeutic/Allergy Injections
The appropriate SIUE therapeutic order form must be completed AND medical records received by the ordering provider.
For Therapeutic injections, please have your provider complete the Therapeutic Injection Orders
For allergy injections, please have your allergist complete the Allergy Injections Order Form
Once SIUE HS has received and reviewed ALL required forms and records, our office will notify the student to schedule an appointment if eligible.
What to Bring to Your Initial Visit
- Medication/Allergy Serum clearly labeled with name and date of birth and any additional supplies
- Student is responsible for storing the medication as directed by the manufacturer and removing from refrigeration if applicable.
- Health Service will ONLY store allergy serums
- Epi-pen (if required)- make sure not expired
Fees for Injections
The student is expected to pay for the therapeutic injection(s) on the same day the service is rendered. The bill may then be submitted to their insurance company. Cost of injections =$10 for one injection and $2 each for each additional injection.
