Hiring Forms
All forms must be returned to eoa-titleix@siue.edu.
- Recruitment Routing Slip – Complete and attach the Notice of Employment Opportunities, advertisements, and their invoice voucher, purchase requisition, or indication of payment by p-card to the Recruitment Routing Slip. Forward all of this information to be approved for posting as indicated on the form.
- Applicant Data Request – As faculty applicants send their application materials, provide each of them with an Applicant Data Request form to be returned to EOA.
- Equal Opportunity Information – Prior to offering employment, EOA must receive this completed recruitment history form.
- Justification in Lieu of Degree – Prior to offering employment, the department leadership, dean or director, and appropriate vice chancellor must approve a candidate who does not meet the minimum qualifications. Once approved in at the division level, EOA must review and approve it as well.
- Search Committee Relationship Disclosure & Conflict of Interest Certification - All members of a search committee must submit this form when the search committee is formed. If the member has a relationship (professional, personal, familial, political, or any circumstances or events involving candidates) with any candidate, they must submit an updated copy of this disclosure to EOA for review.
- Justification of Selection – Once a candidate has been selected, the hiring manager must submit a completed hiring justification, outlining the skills and abilities that led to the selection of a candidate, as well as detailing why any other interviewed candidates were not selected.
- Request for Waiver of Search Process – In alignment with the SIUE Revised Search Waiver Policy, departments can request for a waiver of the search process. The completed form must be approved by the appropriate vice chancellor, Human Resources, and EOA (and the SIU System President if the position is within two reporting lines of the SIUE Chancellor).
