Commitment to Integrity
Per the Conflicts of Interest and Commitment Policy 1Q9 and the Policy on Outside Employment for Faculty, Personnel Policies, SIUE, II 26, employees must ensure that their outside obligations, financial interests and activities do not conflict or interfere with their commitment to the integrity of projects, sponsors, the University and public interest. This obligation pertains to both full-time and part-time employees. This integrated Conflicts of Interest (COI) and Commitment (COC) Disclosure Form collects information related to potential conflicts of interest and commitment, including non-university employment.
The Disclosure Form includes progressive display (i.e., conditional, skip or branch logic), which allows additional sections or questions to populate based on responses to other questions in the form. To log-in, use your SIUE e-ID and password. Please be certain to answer all primary and follow-up questions displayed when completing your disclosure. If you need to exit the form and return to it later, be sure to save the form.
This certification must be updated if anything changes with your COI/COC and, at a minimum, annually for sponsored projects (FCOI).
If you are disclosing nepotism, work with your department chair to complete the template nepotism management plan.
