Reporting Concerns/Incidents
CARE Report
- The purpose of a Care Report is to share mental health concerns or concerning behavior about a student, staff or faculty member with the Dean of Students and Counseling Services for them to review to see what, if any, steps should be taken. If you are concerned for the immediate health and safety of yourself or others, please call 911.
Bias Incident Report
- A "Bias Incident" is defined as an act — verbal, written, physical or psychological — that threatens or harms a person or group on the basis of actual or perceived race, religion, color, sex, age, sexual orientation, gender identity or expression, national origin, ancestry, disability, marital status, civil union status, domestic partnership status, atypical heredity or cellular blood trait, military service, or veteran status. If you are concerned for the immediate health and safety of yourself or others, please call 911.
Sexual Assault, Sexual Misconduct and Relationship Violence Report
- This form can be used by any member of the SIUE community to file a report, either on your own behalf or on the behalf of another person. The Office of Equal Opportunity, Access and Title IX Coordination will review the information you provide, take the most appropriate action, and make the necessary referrals and support. Someone from the above-mentioned office will follow up with you. If you are concerned for the immediate health and safety of yourself or others, please call 911.
SIUE Name Change Process
You can change your name by visiting the Registrar's website. Go to the forms section and under student forms find the Name Change Request Form. If you have gone through the legal process to change your name or if your chosen name is different from the name that was assigned to you at birth, this is the form you will need to complete.

This library guide highlights resources and information relating to LGBTQIA+ individuals. You can use this guide to find articles, associations, books and eBooks, web resources, and more. You can also find information on citing sources.
SIUE Gay/Straight Alliance
The purpose of the Gay-Straight Alliance at SIUE is to allow LGBTQIA+ students and allies the opportunity to meet in both organized and casual settings to promote community, safety, improved mental health and friendship, while also working toward a better campus and community. While our main purpose is not a particularly political one, we strive to promote diversity and inclusiveness, and we encourage our members to make their voices heard in every arena.

SIUE Safe Zone Allies
Safe Zone allies have been trained and are available for all faculty, staff and students who have concerns or questions about LGBTQIA+ topics on our campus.
Counseling Services
Counseling Services provides free, confidential individual counseling for students. Many students dealing with questions or concerns related to sexual orientation or gender identity choose to seek counseling as a safe place for discussion. Counseling Services is a designated Safe Zone dedicated to providing all clients quality care with respect for sexual orientation and gender identity. University Housing offers and unique and varied living experience for all SIUE students. University Housing prides itself on being a progressive department that fosters an inclusive environment for all residents. For members of the LGBTQIA+ and Ally community there are a couple of housing options that may meet your needs better than traditional housing options or living off-campus.
