Arrival at SIUE
- Airport Pick-Up Form - Airport pick-up request only for new international students arriving within the ideal arrival dates. Airport pickup request for Spring 2025 will open in November; keep an eye on your SIUE email for details.
- Check-In Form - Immigration check-in for new international students. New students should fill this out as soon as they land in the United States.
- IHP Student Registration Form - Form for international students requesting an International Hospitality Program host family
Enrollment Requirements
- Graduate Assistant Underload/Overload Approval Form- Graduate School form for graduate students with assistantships to request underload or overload approval
- Leave of Absence/Withdrawal Notification Form - Form to request a leave of absence or withdrawal from classes at SIUE
- Reduced Course Load Form - Form to request approval if enrolled below the minimum required course load
- Returning from a Leave of Absence Form - Form to return from a leave of absence for international students

- Academic/Faculty Advisor Recommendation Form - Recommendation form for I-20/DS-2019 extension Request
- Add/Drop Form - Registrar form to add or drop classes
- Change of Status Request Form - Form to request a U.S. visa status change to F-1
- I-20/DS-2019 Extension Request - Form to request an approval to extend your immigration documents and status to complete your program
- Letter Request Form - Submit a form to request a Driver’s License, Good Standing or Address Verification letter
